Comprehensive Examination: Your initial visit includes a thorough exam and a detailed discussion of your treatment options.

Same-Day Care Options: In many cases, treatment can begin on the same day after we review your treatment options and financial details. Complex or extensive treatment plans may be scheduled across multiple visits to ensure your safety and comfort.
What to Bring to Your First Appointment

Completed Forms (Download Below): Please print and complete the Patient Information Questionnaire and the Patient Information Privacy Form prior to your arrival.

Photo ID: Driver’s license or current photo ID.

Insurance Information: Necessary completed dental insurance forms and card.

Medication List: A full list of any current medications you are taking.

Previous X-Rays: Request that your previous dental office forward your X-rays to us, or bring them with you. Additional digital X-rays can be taken at our office if needed.
